This sounds very interesting! I've read his The Checklist Manifesto, where one of his examples is a surgery he performed that went wrong (and where the patient survived because other members of his surgical team had reviewed their checklists of what needed to be ready just in case, so they were all prepared for the unexpected complications); I'll have to check this one out sometime.
